Thank you for your interest in membership with The Oregon Telecommunications Association. The on-line application process is currently under construction. During this construction phase, you can download the membership application below, then complete and fax or mail it to the OTA office. 	Category I - Telecommunications Provider
			Any telecommunications utility or cooperative providing services within the boundaries of exchange maps filed and approved by the Oregon Public Utility Commission and any individual or business that is authorized to do business as an interexchange carrier, wireless provider, competitive telecommunications service provider, reseller of telecommunications services, shared tenant services provider, cable television provider, radio paging provider, internet service provider, or any other telecommunications provider approved by the Board of Directors.
	Category II - Associate Member
			Any manufacturer, jobber, supplier, service firm, organization or individual who regularly furnishes materials, supplies or services which are used in the operations of the above members or a telecommunications provider that does not provide service in Oregon may join as an Associate Member. This class of membership has one position on the Board of Directors and one vote as a class in matters that come before the entire Association.
	Category III - Premier Associate Member
			The Premier Associate Member demonstrates a high level of support to the OTA and the industry. The Premier Associate Member receives an invitation to the Board Reception at the Annual Meeting; an invitation to the Manager and Committee Chair luncheon held each year; signage at the Annual Meeting indicating your Premier Associate Member status; discounted web site advertising; and priority booth location at the Annual Meeting.
	Conference Attendee
			This is a default member type automatically created by the convention system for non-member convention attendees.
